Accepting pediatric referrals from pilot sites including:
URMC Ambulatory Clinic 6th floor (AC6)
URMC Culver Medical Clinic
Rochester General Pediatrics
Anthony Jorndon Health Center
Unity Health System- Pediatrics

This project is a community wide demonstration project for pediatric asthma patients with persistent asthma at the pilot sites listed. Each practice is conducting intensive Quality Improvement methods that will incorporate the proper treatment of asthma through integration of the National Clinical Guidelines for Asthma Management. In addition, BOH has developed an Asthma Registry to assist practices with key indicators of best practice care and engaged care coordinators to assist families and the clinical teams in implementing these strategies.

This is a 3 year long project with hopes to determine Monroe County Best Practices in Asthma Care at the Primary Care Setting.  Funding was provided through donors to URMC Children's Hospital for the Pediatrics Department. Due to an existing partnership with RCAN and previous successful Asthma projects in the Pediatric Emergency Department, Outpatient Clinic and Inpatient Settings: Hospital administration identified Pediatric Asthma as the focus for these funds.
