1. Do you take your quick- relief inhaler more than TWO times per week?

YES         NO

2. Do you awaken at night with asthma more than TWO times per month?

YES        NO

3. Do you refill your quick-relief inhaler more than TWO times per year?

YES       NO

If you can answer YES to ANY of these questions

ask your Doctor about

"CONTROLLER MEDICINES"

It has been a pleasure to work together with RCAN to Improve Asthma Acare in the Pediatric Emergency Department. The Improvement Process using the Chronic Care Model has been very successfull during our 2008-2009 project. We are excited to see more families leaving the ED with the knowledge and tools they need to better manage their Asthma, recieve the proper Primary Care they need and hopefully the ability to avoid the need to return to ED in crisis.
